Good Faith Estimate
You have a right to receive a Good Faith Estimate of expected charges under the No Surprises Act.
Under the law, we are required to provide patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services scheduled. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, as noted below:
If your scheduled appointment is:
- 10 or more days out, your estimate will be ready within 3 days
- Within 3-9 days, your estimate will be ready the next business day
- In less than 3 days, we are not required to provide a GFE
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Sliding Fee Scale
This agency serves ALL patients, regardless of ability to pay.
The Sliding Fee Discount Program is available to all patients.
Discounts are offered based on household income and family size.
